INFORMED CONSENT: INSTALLATION AND OPERATING CONDITIONS

Equipment: HERO Purification System
Date: March 25, 2026
Client: Enrique Contreras

1. DECLARATION OF PREEXISTING CONDITIONS

The Client acknowledges and agrees that the property where the installation will take place presents the following technical conditions, which may affect the system's performance:

  • Condition of the Pipes: It has been identified that the home has issues with its piping system.
  • Water Quality: The incoming water is contaminated, increasing the load of sediments and residues that the purifier must process.
  • Water Analysis Results: The study showed Iron at 7.5, high levels of Tanins, presence of Sulfur, and a TI10 reading above 1200 solids. According to the EPA, water with a solids indicator above 500 is NOT CONSIDERED HEALTHY WATER.
  • Recommendation: Reverse Osmosis for the entire home.

2. TECHNICAL COMMITMENT AND MAINTENANCE

The HERO purification system will include:

  • 1 retention tank of 120 gallons
  • 1 solution tank (chlorinator)
  • 1 Big Blue 21" filter
  • 1 salt tank
  • 2 systems with components for controlling bad odor (sulfur) and iron
  • Water softening, purification, and filtration processes

Due to the conditions described above, it is established that:

  • Additional Maintenance: The client accepts that these external conditions may require preventive maintenance beyond standard procedures to guarantee proper water flow and purity.
  • Function Warranty: The HERO system is recommended because, with maintenance performed at the scheduled intervals and the appropriate level of care, it will operate effectively and provide high-quality purified water. OZONE WATER TREATMENTS is responsible for the installation process, provided that the home’s pipes are in optimal condition. Once the system is installed, WE DO NOT GUARANTEE THAT THE PROBLEM WILL BE ELIMINATED 100%, as this will depend on the client’s preventive care and maintenance.
  • Client Responsibility: The client accepts responsibility for replacing any components necessary for the proper functioning of the system. The first months and the recommended maintenance schedule will determine how often preventive care should be performed on the following components: Big Blue filter, baskets, pistons, salt replenishment, and chlorine replenishment for system wash cycles.

3. MANDATORY FOLLOW-UP PLAN

Due to the high concentration of sediments, the optimal functioning of the equipment strictly depends on a preventive maintenance schedule. The following is agreed:

  • Frequency: Follow-up visits and technical cleaning will be carried out every 5 months.
  • Duration of the Plan: This intensive monitoring will remain in effect for 2 years from the installation date.

4. CLIENT RESPONSIBILITY

The Client agrees to:

  1. Comply strictly with the preventive maintenance schedule.
  2. Provide the necessary care for the system to protect its internal components from saturation caused by external sediments.
  3. Understand that this process is carried out in coordination with the credit institution Time Investment, and that no issue related to the functioning of the system will serve as grounds to avoid the agreed monthly credit payments.

5. ACCEPTANCE AND SIGNATURE

By signing this document, the Client acknowledges having been clearly informed about the condition of the hydraulic networks and accepts that the optimal performance of the HERO system depends on adherence to the scheduled technical visits.

Enrique Contreras.
